Day in and day out, medical-grade and industrial oxygen cylinders and pressure systems hold enormous amounts of energy.

These are crucial pieces of hardware doing valuable work, but it does mean that when things go wrong the outcome can be explosive.

Let’s take a look at one potential vulnerability when it comes to oxygen storage, and that’s the filling systems that oxygen cylinders need to connect to for refilling.

After a recent incident, the oxygen safety and testing specialist WHA International was called out to explore what went wrong and to draw lessons for the future. Its investigation highlighted several operational and design issues.
